Some undesirable practices and misapplications that caused valve-related failures are examined, and future courses of action are recommended to avoid repetition of these events. Desirable valve characteristics and practices that should be considered when selecting valves for use in tritium service are also discussed. Desirable valve and system features and operational actions are grouped into two categories: strongly recommended and recommended.
